At Empower Path Homes, we deliver safe, trauma-informed, and outcomes-driven transitional housing for two priority populations:

  • Women with infants under 5 months, including those in crisis or reentry, in need of immediate stabilization

  • Aging adults in transition, seeking independence in a supportive, structured environment

 

Headquartered in Northern Kentucky and serving the Greater Cincinnati area, our homes are designed for restorative living — offering private and shared rooms, family-friendly accommodations, and wraparound coordination with community service providers.

We specialize in serving:
✔ Women impacted by incarceration, substance use, or housing insecurity
✔ Mothers with infants who need a safe, private, and structured recovery environment
✔ Justice-involved adults seeking to reintegrate with stability
✔ Older adults discharged from congregate care, hospitals, or shelters

We do not offer clinical or licensed medical services onsite. Instead, we act as a housing-based service partner, coordinating closely with case managers, reentry specialists, and MCOs to ensure continuity of care and long-term housing success.

Empower Path Homes was founded by a purpose-driven leader with a background in workforce development and a deep commitment to creating stable, supportive housing environments that foster structure, safety, and second chances. Led by a culturally conscious, faith-driven team with lived experience and strategic insight, Empower Path Homes designs housing programs that address critical service gaps — with dignity, structure, and cultural relevance at the forefront.

Our homes are not shelters — they are stabilization zones.

Our partners are not vendors — they are collaborators in long-term impact.

We serve two core populations through private-pay and referral partnerships:

  • Women with infants, including those exiting crisis, incarceration, or unstable housing

  • Aging adults, including justice-involved or medically vulnerable individuals needing structured, non-medical support

 

We collaborate with:

  • Case managers, probation officers, and reentry programs

  • Managed Care Organizations (MCOs) and behavioral health agencies

  • Hospitals, shelters, treatment centers, and nonprofit partners

  • Partner for outcomes — housing retention, recovery stability, and reintegration

Our Services

1

Housing for Women
(Up to 18 Months)

Who It's For:

  • Women in transition from jail, rehab, or unstable housing

  • Mothers with infants (under 5 months)

  • Clients needing a sober, supportive, and structured environment.

Includes:

  • Fully furnished shared housing

  • Routine-based daily schedule with curfews

  • Mentorship and accountability

  • Connection to resources (employment, childcare, benefits)

  • Life skills coaching.

Independent Living for Aging Adults (55+)

Who It's For:

  • Older adults who are low-income or justice-involved.

  • Individuals being discharged from hospitals or long-term care.

  • Clients who need stability but not clinical services.

Includes:

  • Routine wellness checks and community meals.

  • Fully furnished shared housing.

  • Transportation assistance (non-emergency, scheduled).

  • Light housekeeping and support with basic tasks.

  • Linkage to primary care and benefits navigation.

What We Don’t Provide:

  • We are not a licensed clinical or medical provider.

  • We do not offer detox, skilled nursing, or behavioral health treatment on-site.

  • We partner with agencies that do.

How it Works

1. Referral or Inquiry Received
We accept referrals from case managers, hospitals, re-entry programs, adult protective services, and family advocates. Self-referrals are also welcome on a case-by-case basis.

2. Eligibility & Intake Screening
Our team conducts a brief intake screening to determine program fit, verify housing needs, and ensure safety alignment with our community standards.

3. Placement in Housing
Once accepted, residents are placed in a fully furnished, shared home. Each home includes utilities, Wi-Fi, basic furnishings, and access to routine support services.

4. Resident Orientation & Welcome Plan
We walk each new resident through house expectations, community rules, and a basic schedule to help them transition smoothly. We provide a Welcome Packet and assign a primary point of contact for any support questions.

5. Ongoing Support & Check-Ins
While we are a non-clinical housing provider, we offer optional wraparound add-ons such as peer recovery coaching, transportation coordination, meal plans, and care coordination. Monthly check-ins and wellness calls are built into our housing model.

6. Program Transition or Step-Up Pathways
We work closely with case managers and referring agencies to monitor progress and assist with the resident’s long-term housing goals, such as stepping into independent apartments or higher care levels.
